“This office closed without the courtesy of notifying their patient's or referring them elsewhere. One day patients were seen for their appointment, copay was taken but their prescriptions were never called in. When you tried to call the office no one answered and the voice mail was full and you couldn't leave a message. This caused many to go through withdrawals from the pain medications and some patient's ended up in the hospital. People continued to show up for their appointment only to find a sign on the door that said " Closed "Permantley". Very poor practice and spelling!!”
