“I've had some problems that still haven't been resolved. We came to an agreement and I still haven't got me medication issue fixed. As it stands I'm not going to have my 30mg IR after Saturday 3/22/25. Got the correct amount for April 7th & May 7th, according to the pharmacy. I have the correct amount of pills for the 30mg ER but was only given half the amount for the 30mg IR. Spoke with your nurse Ashley, on Thursday 3/19/25, which she stated was going to have my partial amount of 30mg IR sent to McCoy & Tygart as of today 3/21/25 at noon the pharmacy said they haven't received any request for this. As I stated to you if you come across a medical option that you feel could really help me , I'll trust you and go forward with that. I just need my medication corrected. Since May 18, 2024, I've had 5 casts, endured 4 surgeries, one thumb with 2 bolts and fusion that wasn't healing, hence the 5 casts of which in January 2025 was released from doctors care. Had 3 cataract surgeries of which I just got released on 3/13/25, Thursday. Just started back exercising at the gym this week. Being that it's been 11 months since I've done this I've been in more pain because muscles getting used to exercising. I'm doing light workouts looking to increase to my normal weights before all these medical issues. I do trust you have my best interest. I only ask to have my medications resolved. Thank you. Kindly Augustine Montiho III”
