“Nice guy. That's where it ends. Had a good experience 13 years ago, but not this time. No IV sedation for any of his shot treatments. When the shots fail, your out of luck. This doctor does NOT prescribe any meds, not even gabapentin!! Not even a narcotic, but he refused to write any. He sends a "referral" to your primary doctor who has the option of not prescribing it for you. And most won't because Dr Culver office MAKES you sign that form on narcotics, which means you can't have any other doctor prescribed meds for you. Load of BS. Just in it to bill your insurance”
